The official NWS report is out on the Hallam, NE tornado on 5/22/04. It
has been rated an F-4 on the Fujita Scale.

They note: "MAXIMUM PATH WIDTH: 2 1/2 MILES". If I'm not mistaken, that
could be the widest ever.
The Tornado FAQ from the master Roger Edwards says "What was the biggest
known tornado? Fittingly, it was in Texas -- specifically, in the high
plains of the Texas Panhandle near Gruver on 9 June 1971. At times, the
tornado was over 2 miles wide, with an average width of about 2500 yards."
